Inclusion criteria

Inclusion criteria: 1. Age >= 60 years old; 2. Diagnosed with type 2 diabetes >= 1 year and glycation >= 7%; 3. Live in your own residence or with friends or relatives (not in a nursing home, nursing home, rehabilitation hospital, or other long-term care); 4. Clear consciousness, able to act independently, and can complete the assessment by themselves or with the explanation of the staff; 5. Patients and their family members have good compliance, voluntarily participate in the research and sign the informed consent.

Exclusion criteria

Exclusion criteria: 1. Accompanied by acute complications of diabetes such as severe infection, diabetic ketoacidosis, hyperosmolar hyperglycemia syndrome, etc.; 2. Previous cerebrovascular accidents and vascular cognitive impairment; 3. Severe cognitive, vision, hearing, language impairments, etc. cannot complete the contents of the questionnaire; 4. End-stage patients (life expectancy less than half a year); 5. Other conditions that the researcher thinks are not suitable for participating in this study.
