Inclusion criteria
Inclusion criteria: 1. patients with clinically confirmed chronic kidney disease (CKD); 2. Agree to participate in chronic disease management program and sign informed consent; 3. Aged between 18 and 80.
Exclusion criteria
Exclusion criteria: 1. Pregnant or breast-feeding; 2. Any form of drug abuse, mental illness, or other conditions that the investigator considers may affect the results of the study or the patient's physical health; 3. The investigator judged that the patient was not suitable for the study.
