Inclusion criteria
Inclusion criteria: 1.Patient (1) male or female between 18 and 70 years old; (2) be firstly diagnosed with ischemic or hemorrhagic stroke either by computerized tomography scan or magnetic resonance imaging; (3) more than 6-month history after stroke; (4) Modified Rankin Scale (mRS) score of two to four points; (5)be at a home rehabilitation period and there is no admission plan within three months; (6)residence within or around the city of Shijiazhuang. 2.Caregivers (1) male or female aged 18 or over; (2) be the primary caregivers and can complete a 3-months trial; (3) have a certain cultural foundation and be skillful at operating WeChat.
Exclusion criteria
Exclusion criteria: 1. score below 15 points on the Glasgow Coma Scale score(GCS); 2. have cognitive and(or) psychotic disorders; 3. have severe comorbidities such as circulation, digestive, immune, and blood systems disorders; 4. exist other diseases of the locomotor system which will influence survivors' motor function such as fracture, severe osteoporosis, and osteoarthritis; 5. have bilateral brain lesions; 6. have severe sensory dysfunction.
