Inclusion criteria
Inclusion criteria: Patients: ?Age =60 years old; ?Have lived in urban communities in Fuzhou for =6 months; ?Have lived with family members; ?Have been diagnosed with hypertension or diabetes by secondary or above hospitals. Family caregivers: ? Aged = 18 years old; ? Willing to participate in this study and able to communicate normally; ? Family members who live together with elderly patients with chronic diseases and undertake their main daily care work; ? If the patient has several families at the same time The caregiver takes care of the family member with the longest caring time.
Exclusion criteria
Exclusion criteria: Patients: Patients with hearing impairment, speech impairment, cognitive impairment, mental illness, severe or terminal illness. Family caregivers: ? Those with cognitive or mental disorders; ? Those with limited mobility.
