Inclusion criteria
Inclusion criteria: Cross-sectional study stage: 1. Meet the 1999 WHO diagnostic criteria for type 2 diabetes. 2. Have lived in the community health service center of this survey for a long time. 3. Have complete cognitive and behavioral abilities. 4. Informed consent to participate in this study voluntarily. Randomized controlled intervention phase: 1. It meets the 1999 WHO diagnostic criteria for type 2 diabetes. 2. Long-term resident in Xietu Street Community Health Service center area. 3. Have complete cognitive and behavioral abilities. 4. Informed consent to participate in this study voluntarily. 5. Have a regular roommate.
Exclusion criteria
Exclusion criteria: Cross-sectional study stage: 1. Patients with severe physical impairment; 2. Severe audio-visual impairment; 3. With conditions that may lead to non-compliance and loss to follow-up; 4. Currently participating in or have participated in other related research projects within the recent 30 days. Randomized controlled intervention phase: 1. Patients with mental disorders and Alzheimer's disease who cannot cooperate; 2. Patients with severe physical impairment; 3. Persons with severe audio-visual impairment; 4. There are circumstances that may lead to non-compliance or loss of follow-up of participants; 5. Participants are currently participating in or have participated in other relevant research projects within the last 30 days.
