Inclusion criteria
Inclusion criteria: 1. No gender limit, age 40 to 80 years old; 2. A history of moderate or severe AECOPD >= 1 time in the 12 months before enrollment, and no AECOPD events occurred within 4 weeks before enrollment; 3. After training, they can pass the "Internet Cognitive Level and Mobile Device Experience" test.
Exclusion criteria
Exclusion criteria: 1. Moderate to severe functional impairment of any one of liver, kidney, and heart (ALT and Cr exceed the normal upper limit by one time, and NYHA reaches Grade IV); 2. mMRC level 4 (inability to leave home due to severe breathing difficulties, or difficulty breathing while dressing or undressing); 3. Long term bed rest (more than half of the time spent awake); 4. Other active diseases (such as acute or chronic infectious diseases, incurable malignant tumors, etc.); 5. Pregnancy or planned pregnancy within one year; 6. Difficulty in follow-up (unable to visit hospitals as needed, unable to contact remote doctors as needed).
