Inclusion criteria
Inclusion criteria: 1. 65 years old and above, gender is not limited; 2. Suffering from at least one chronic disease (judged by ICD9 diagnosis code); 3. Concomitant medication >= 5 kinds, taking the medication for at least half a year; 4. Have at least three outpatient follow-up visits in registered medical institutions within one year; 5. The patient himself signed the informed consent.
Exclusion criteria
Exclusion criteria: 1. Cannot or refuse to use smart phones and other devices; 2. Tumor patients; 3. Patients with organ or hematopoietic stem cell transplantation; 4. Mental illness or severe cognitive problems that may prevent participation in the project; 5. Poor compliance, difficult to cooperate with research intervention and follow-up; 6. Participated in other clinical research in the last 3 months; 7. Other researchers think that it is not suitable to participate in the research.
