One-lung ventilation in thoracic surgery

Interventions

Group HPC:After confirming the proper position of DLT and before OLV, the planned surgical lung (non ventilated side lung) was intermittently ventilated for three cycles, each cycle including opening
Group AOI:After OLV, the lung on the operation side (non ventilated side) was blown with oxygen at the speed of 3 L / min for 60 minutes
After the oxygen inhaler, humidification bottle and disposable oxygen delivery pipe of the hanging tower in the operating room are connected in turn, the DLT lumen
Group CON:For the non-ventilated lung, neither preoperative hypoxic preconditioning, nor intraoperative breathing and oxygen blowing.

Inclusion criteria

Inclusion criteria: 1.18-79 years male or female; 2.Thoracic surgery with one lung ventilation is planned; 3.ASA I-III; 4.The estimated time of mechanical ventilation = 2 hours, and the time of one lung ventilation = 1 hour.

Exclusion criteria

Exclusion criteria: 1. Body mass index>35kg/m2; Previous lung surgery history; 2. Previous history of chest chemotherapy or radiotherapy; 3. Severe obstructive or restrictive pulmonary disease; 4. Bronchial infection or bronchopleural fistula; 5. Sleep apnea syndrome; 6. Severe hypertension and cardiovascular disease; 7. Liver or renal insufficiency or left ventricular insufficiency; 8. Arrhythmias affecting hemodynamic stability; 9. Known or expected airway difficulty or tracheotomy; Anemia (hemoglobin<100 g/L); 10. Peripheral blood oxygen saturation (SpO2)<94% when breathing indoor air; 11. Mechanical ventilation within 2 weeks before operation; 12. Pregnancy; 13. Refuse to participate or fail to cooperate in completing the study.
