Inclusion criteria

Inclusion criteria: 1. Conform to the diagnosis of chronic heart failure and the New York Heart Association (NYHA) Class II-IV standard for cardiac function; 2. There is no instrument treatment plan for heart failure within 1 year after discharge; 3. Age = 18 years old; 4. Agree to participate in this study.

Exclusion criteria

Exclusion criteria: 1. Have a history of acute myocardial infarction within 1 month before enrollment; 2. People suffering from other serious diseases or malignant tumors of the system; 3. Those with other systemic diseases that may lead to repeated hospitalization are merged at the same time; 4. Patients with valvular heart disease requiring surgical treatment; 5. People with cognitive and mental disorders.
