Inclusion criteria

Inclusion criteria: 1. Patients who first recorded a diagnostic read code for hypertension, diabetes, coronary artery disease, chronic kidney disease in the electronic health record of the hospital information system between April 1, 2011 and March 31, 2021. 2. Have their official health records in the system, and conduct continuous care follow-up and management of patients, including basic demographic information, clinical data and laboratory testing data.

Exclusion criteria

Exclusion criteria: 1. Patients with a life expectancy of less than 2 years due to age-related complications, such as cerebral infarction or pancreatic cancer; 2. Patients younger than 35 years at the time of diagnosis.
