Interventions

Intervention Group:Routine nursing measures include distributing health manuals, regularly measuring weight and blood pressure, oral diet and exercise and other health education
Intervention Group:Establish a health management team for GDM high-risk population led by specialist nurses. Risk assessment is carried out according to the risk factors of pregnant women themselves,
:Develop individualized diet and exercise programs for pregnant women according to their own characteristics
Intervention Group:emind pregnant women to measure their weight every day and draw a graph to visually display the weight gain during pregnancy
Intervention Group:Choose a daily exercise and clock in based on one's preferences and habits
Intervention Group:Push relevant health information to make pregnant women aware of the dangers of gestational diabetes and the importance of prevention and self-management knowledge

Inclusion criteria

Inclusion criteria: 1. Pregnant women with gestational age less than 12 weeks and single pregnancy; 2. Pregnant women who have been filed in Beijing Friendship Hospital and can have regular antenatal examination for a long time; 3. Be able to use smart phones; 4. Any one of the high-risk factors of diabetes in pregnancy, including age = 35 years, pre pregnancy body mass index>25kg/m2, family history of diabetes or pregnancy diabetes, previous history of macrosomia (newborn weight = 4000g), and patients with polycystic ovary syndrome.

Exclusion criteria

Exclusion criteria: 1. diabetes diagnosed before pregnancy or diabetes found in early pregnancy; 2. People with serious liver, kidney, heart and brain diseases in the past; 3. Those with threatened abortion or obstetric complications who are unable to perform activities; 4. Those who participate in other research projects or are unwilling to cooperate with the intervention.

OGTT test at 24 weeks of pregnancy fasting blood glucose, 1 hour, 2 hours after the average blood glucose;Gestational weight gain at 36 weeks;Complications during childbirth: cesarean section rate, premature rupture rate, neonatal asphyxia rate;The proportion of insulin use in pregnant women with GDM, the proportion of hospitalization in pregnant women with poor blood glucose control, and the length of hospitalization;Neonatal outcome: neonatal weight, the proportion of large for gestational age infant ,Apgar score at birth, number of cases of hypoglycaemia;Women’s Self-management ability score and self-efficacy level;
