Inclusion criteria
Inclusion criteria: 1. Aged > 18 years; 2. Experience of being discharged from hospital after stroke and living at home for at least > 2 weeks; 3. Motor dysfunction of different degrees caused by stroke; 4. The informed consent signed by the patient himself or by his or her immediate family members is voluntary to participate in the study.
Exclusion criteria
Exclusion criteria: 1. Sequelae caused by other encephalopathy, such as tumor, brain trauma, cerebral parasitic disease, etc.; 2. Stroke patients with no sequelae of neurological impairment, such as transient ischemic attack (TIA); 3. Patients with speech comprehension disorder or severe auditory or visual impairment; 4. The condition is unstable, the body is weak, can not tolerate and other reasons can not cooperate.
