Inclusion criteria
Inclusion criteria: 1. Community Health Service Center in Yuexiu District; 2. Community health service centers who are willing to participate in the community standardized anticoagulation management program for atrial fibrillation; 3. Patients with confirmed valvular or nonvalvular atrial fibrillation (duration of atrial fibrillation >= 30 seconds recorded by ECG or Holter); 4. Patients or family members are willing to participate in the AF health management group and communicate with each other through wechat group; 5. Sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Community health service centers that have conducted pilot community standardized anticoagulation management projects for atrial fibrillation in the early stage; 2. Patients with AF caused by reversible factors, such as hyperthyroidism, recent myocardial infarction, pulmonary embolism, surgery, etc.; 3. Patients with a life expectancy of less than 1 year.
