Inclusion criteria

Inclusion criteria: 1. Patients aged 18-70 years; 2. Patients who agreed to receive online diabetes self-management support under the Internet + diabetes co-care model; 3. Patients with type 2 diabetes mellitus; 4. Voluntarily sign the informed consent before conducting the study.

Exclusion criteria

Exclusion criteria: 1. Patients who cannot cooperate and complete the study; 2. Patients with psychological and mental disorders judged by doctors.
