Inclusion criteria

Inclusion criteria: 1. Age > 45; 2. Residents who are willing to participate in research and sign informed consent forms; 3. People at high risk of stroke (atrial fibrillation, atherosclerosis, hypertension, TIA, application of the China-PAR model to assess a 10-year risk of cardiovascular disease >= 10% or a history of stroke); 4. Ability to use your smartphone autonomously or with the help of your family.

Exclusion criteria

Exclusion criteria: 1. Pregnant woman; 2. Patients with malignant tumors.
