Inclusion criteria
Inclusion criteria: 1. Aged >= 60 years, including the elderly diagnosed with diabetes before and after the age of 60; 2. Patients who meet the diagnostic criteria for diabetes proposed by WHO (1999), are clearly diagnosed with type 2 diabetes, and are included in the community chronic disease management system; 3. Able to take care of themselves in life and participate in health education lectures; 4. Able to communicate verbally and have a certain ability to understand the content of education; 5. Stable condition without severe coronary heart disease, hypertension, liver and kidney insufficiency, poor lung function and bone and joint disease; 6. Voluntarily participate in this research, and it is expected that the participation in this research will not be interrupted within one year due to reasons such as going abroad or vacation.
Exclusion criteria
Exclusion criteria: 1. Patients with serious complications of diabetes, such as diabetic foot or physical disability; 2. Accompanied by severe heart, brain, kidney, liver and respiratory system diseases or bone and joint diseases, with a life expectancy of less than two years; 3. Patients who participate in other research projects at the same time.
