Inclusion criteria
Inclusion criteria: 1. Targets of basic public services for chronic diseases, permanent residents aged 35-70 years are preferred; 2. Pre-diabetic patients, diagnosed in accordance with the ''China Guidelines for the Prevention and Treatment of Type 2 Diabetes (2018 Edition)'', have received diabetes screening within the last year, and residents diagnosed with pre-diabetes are preferred; 3. Prioritize having one of the following modifiable risk factors: (1) Overweight or obese (BMI >= 24); (2) Central obesity (waist circumference >= 90cm for men and >= 85cm for women); (3) Hypertension (systolic blood pressure >= 140 and (or) diastolic blood pressure >= 90); 4. Sign the written informed consent and are willing to participate.
Exclusion criteria
Exclusion criteria: 1. Patients with severe acute and chronic complications, serious chronic diseases of various important organs, tumors, acute and chronic infectious diseases, physical mobility disorders, and any mental illness; 2. Patients who are pregnant or breastfeeding; 3. Those with a history of alcoholism or illegal drug use; 4. Those who are unwilling to cooperate with intervention projects; 5. Patients who need to be hospitalized; 6. The patient has a history of serious cardiovascular events (myocardial infarction, stroke, acute coronary syndrome and transient ischemic attack (TIA).
