Inclusion criteria

Inclusion criteria: 1.Patients >= 18 years; 2.The main diagnosis was acute cerebral infarction; 3.Cerebral infarction confirmed by CT or MRI; 4.Telephone follow-up can be completed; 5.Patients took medicine when they were discharged from the hospital.

Exclusion criteria

Exclusion criteria: 1.Presence of hemorrhagic stroke or other bleeding diseases; 2.Patients have significant aphasia/dysarthria; 3.Modified Rankin Scale (MRS) of > 4 at discharge; 4.With dementia or cognitive impairment.
