Inclusion criteria

Inclusion criteria: 1. Meet the guideline diagnostic criteria for COPD and are in a stable phase; 2. Aged >= 60 years old; 3. Can cooperate with the completion of post-hospital follow-up guidance and evaluation; 4. The patient has the ability to use a smartphone; 5. Not receiving the guidance of COPD self-management intervention system; 6. Voluntarily participate in this study and sign the informed consent.

Exclusion criteria

Exclusion criteria: 1. Orthopedic surgery; 2. Neurological and mental problems cause dysfunction or inability to cooperate; 3 unstable heart disease; 4. Severe pulmonary hypertension; 5. Combined with other lung diseases that interfere with the study.
