Inclusion criteria

Inclusion criteria: 1. Outpatient patients; 2. Aged <=18 years old; 3. At least one chronic disease; 4. At least one drug for the chronic disease.

Exclusion criteria

Exclusion criteria: 1. Patients suffering from serious illnesses, confusions or mental disorders that lead to the inability to communicate effectively; 2. Incomplete collection of clinical data that lead to the inability to conduct statistical analysis; 3. The children or their guardians refuse to participate in the investigation; 4. Non-primary health care institution should exclude the diseases that the primary health care institutions have no diagnosis and treatment ability.
