Inclusion criteria

Inclusion criteria: 1. Patients with ischemic stroke who initiate secondary prevention; 2. combining more than 1 kind of chronic disease or complications; 3. The number of conventional drugs >= 3 ; 4. Poor drug compliance.

Exclusion criteria

Exclusion criteria: 1. Cognitive dysfunction; 2. combining several systems of serious diseases; 3. Tumor patients; 4. Unable to cooperate with the completion of telephone follow-up and written records.
