Inclusion criteria

Inclusion criteria: People with clinical diagnosis of dementia, unplanned hospital admission, and medically fit for discharge. He / she would be medically stable without infection risk to others.

Exclusion criteria

Exclusion criteria: People with significant psychiatric problems which may harm himself / herself or other care recipients, in regular need for complicated or risky medical procedures e.g. dialysis, airway suction, tracheostomy which JCCPA may not be able to accommodate with current and proposed capacity, care recipients whose family caregivers have no intention to take care of them at home, old age home residence for more than three months, those who are not able to travel to JCCPA.
