Inclusion criteria
Inclusion criteria: 1. 65 years old and above, regardless of gender; 2. Taking more than 5 kinds of drugs at the same time; 3. The patients have clear consciousness and can communicate with each other effectively; 4. Willing and able to follow up evaluation; 5. Signed the informed consent.
Exclusion criteria
Exclusion criteria: 1. The patient asked to terminate the treatment or died and could not be evaluated; 2. The lack of clinical data can not be counted; 3. Participating in other clinical studies in the last three months; 4. Other researchers think it is not suitable to participate in the study.
