Inclusion criteria
Inclusion criteria: 1.adult (age = 18 years) patients with type 2 diabetes, with hemoglobin A1c (HbA1c) = 7.0% in the last 3 months; 2.can use a smartphone; 3.willing to install a diabetes app; 4.Available for telephone follow-up; 5.Willing to provide written informed consent.
Exclusion criteria
Exclusion criteria: Patients with a life-limiting condition (such as malignant tumors, end-stage renal disease, blindness, and heart failure), cognition impairment, mental health condition, and presence of significant hypoglycemic events and those who have problems in communication (hearing or visual impairment) will be excluded because such conditions may threaten the patients engagement in diabetes self-management activities. Patients who are participating in other interventional studies will also be excluded due to the potential confounding effect.
