Inclusion criteria
Inclusion criteria: 1. Patients with chronic diseases and aged over sixty years. 2. Patients who sign informed consent voluntarily and cooperate to complete the study are eligible. 3. Patient has lived in the local area for more than 36 months.
Exclusion criteria
Exclusion criteria: 1. Patients who have been diagnosed with serious life-threatening diseases with their life expectancy less than 6 months; 2. Patients who are in acute exacerbation of the disease or unable to complete the questionnaire and physical test.
