Inclusion criteria

Inclusion criteria: 1.Dialysis unit: more than 20 maintenance hemodialysis patients; 2.Aged from 18 to 70; 3.Be able to read, understand and complete the questionnaire survey; 4.Maintenance hemodialysis patients in dialysis center: regular dialysis three times a week, each dialysis time is 4 hours, and blood phosphorus > 4.5mg/dl; 5.Hemodialysis time >= 3 months; 6.Willing to participate in the study and follow up, and sign the informed consent; 7.The intervention group will be willing to adjust the medication according to the pharmacist's advice. 8.The flow rate of dialyzer is above 230ml / min.

Exclusion criteria

Exclusion criteria: 1.Patients with mental retardation, language barrier and other factors that affect communication; 2.There was no detection of serum calcium / phosphorus / iPTH three months before enrollment; 3.Those who can't check blood calcium / phosphorus index at least once a month and iPTH every three months; 4.HDF > once a week; 5.Those who have undergone parathyroid surgery.
