Inclusion criteria

Inclusion criteria: 1. Patients older than 18 years old; 2. There are long-term oral drugs outside the hospital; 3. He was previously diagnosed with any disease of hypertension, diabetes or coronary heart disease.

Exclusion criteria

Exclusion criteria: 1. Those who are unable to provide complete medication information due to language, hearing and intellectual disabilities; 2. Patients who cannot provide the medication list, nor can their family members provide; 3. Severely ill patients.
