Inclusion criteria
Inclusion criteria: Aged >= 65 years; Estimated survival time > 3 months; Length of Hospital stay >= 4 days; Barthel Index >= 61 points (ie basic self-care in daily life); existing nutritional risk, which is, the Chinese version of mini nutritional assessment short-form (MNA-SF) is 0-11 points; the patient (or family member) voluntarily participates and signs the informed consent form.
Exclusion criteria
Exclusion criteria: Admitted to the hospital due to anorexia nervosa, acute pancreatitis, acute liver failure, cystic fibrosis or stem cell transplantation; suffering from acute infectious diseases or chronic wasting diseases, such as tuberculosis, malignant tumors, HIV infection, etc.; suffering from heart, lung or People with any life-threatening serious diseases in other organs; severe chronic gastrointestinal diseases; patient suffering from schizophrenia, depression and other mental illnesses; patients with persistent disturbances in consciousness or communication, unable to communicate effectively and their caregivers are unable to provide effective information; Those who have participated in clinical trials within 3 months; patients who have participated in the modeling of the first phase of the study; patients hospitalized in the intensive care unit (ICU), cardiovascular care unit (CCU), Medical intensive care units (MICU), and respiratory intensive care unit (RICU).
