Inclusion criteria

Inclusion criteria: 1. Aged >= 18 years; 2. Presence of CSVD markers on MRI, shows any of the following: 1) White matter hyperintensities (WMHs), Fazekas score 1 >= 2; 2) Fazekas=1 with >= 2 vascular risk factors (hypertension, hyperlipidemia, diabetes, obesity, current smoking, past medical history of vascular events except stroke); 3) Fazekas=1 with lacuna; 4) Recent subcortical lacunar infarction based upon imaging; 3. Independent in activities of daily life (modified Rankin Scale (mRS)<= 2); 4. Provide written informed consent. Note: The imaging definition of CSVD refers to the STRIVE Guideline. 1. The total Fazekas score is 6 which is the sum of Fazekas scores of subcortical and periventricular white matter lesions; 2. Recent subcortical lacunar infarction: Hyperintensity (<20 mm in diameter) in the subcortex, basal ganglia or brain stem on diffusion-weighted imaging (DWI) (restricted diffusion on ADC sequence) with or without clinical symptoms; Hyperintensity (< 20mm in diameter) in the subcortex, basal ganglia or relevant parts of the pontine on fluid-attenuated inversion recovery (FLAIR) imaging of brain MRI with new clinical symptoms.

Exclusion criteria

Exclusion criteria: 1. Acute cerebral infarction with the lesion showing high signal on DWI phase and the diameter > 20mm; 2. Acute cerebral hemorrhage; 3. Acute subarachnoid hemorrhage or previous history of cerebrovascular malformation, aneurysmal subarachnoid hemorrhage or untreated aneurysms (> 3mm in diameter); 4. Diagnosed neurodegenerative diseases (e.g. AD, PD); 5. Presence of non-vascular white matter lesions, for example, multiple sclerosis, adult white matter dysplasia and metabolic encephalopathy; 6. Diagnosis of mental disorder according to the 5th edition of Diagnostic and Statistical Manual of Mental Disorders (DSM-V) criteria; 7. Contraindications of Magnetic Resonance Imaging (MRI) examination (e.g. claustrophobia); 8. Severe organic disease, such as malignant tumor, with a life expectancy less than 5 years; 9. Unable to follow up due to geographical or other reasons; 10. Participate in other clinical trials at the same time.
