Inclusion criteria
Inclusion criteria: 1) People with local household registration or resident time >= 6 months, who are not expected to leave their current residence in the next 3 years. 2) Aged 50-70 years. 3) Primary school or above and have the ability to read and write proficiently. 4) Included in the public heath management system of the community health service center or local health center. 5) Own and use a smart phone (able to use WeChat). 6) Participate in the study voluntarily and sign the informed consent. 7) CHINA-PAR was assessed as ASCVD high-risk population and intermediate risk population with high scores.
Exclusion criteria
Exclusion criteria: 1) Diagnosed with cardiovascular diseases, including stroke, coronary heart disease or myocardial infarction, or severe cardiac insufficiency (NYHA grade III or IV). 2) Having various malignant tumors. 3) Having mental disorders or alcoholism, have no self-control and cannot express themselves accurately;Inability to cooperate (cognitive impairment, physical and other obvious disorders). 4) Definitely diagnosed COPD and cor pulmonale. 5) Having secondary hypertension, thyroid, hypothalamic pituitary gland, adrenal gland and other endocrine diseases. 6) Having diseases requiring urgent treatment: such as acute infection, hypertensive crisis, diabetes mellitus complicated with ketoacidosis, etc. 7) Having severe abnormal liver and kidney function, autoimmune disease, or any serious and fatal disease. 8) Diagnosed with arteriosclerosis obliterans. 10) Having limitation of physical activity due to various causes. 11) Having Parkinson's disease, serious nervous system disease. 12) Having participated in its cardiovascular disease intervention trial.
