Inclusion criteria

Inclusion criteria: 1. Subjects over 60 years old; 2. Patients who were identified as high risk after screening by "simplified risk scoring system for unplanned; readmission of stroke patients"; 3. Subjects who were mainly diagnosed as ischemic stroke; 4. Those subjects who have informed consent are willing to participate in this study.

Exclusion criteria

Exclusion criteria: 1. Patients who died during the first hospitalization; 2. Patients with other serious physical diseases (such as malignant tumor, severe cardiopulmonary complications); 3. Patients with severe mental illness or cognitive impairment and unable to cooperate.
