Inclusion criteria

Inclusion criteria: 1. Aged 60 years or above; 2. Resident residents who have lived at home for more than 6 months in the surveyed community; 3. Has been diagnosed with a chronic disease or reported chronic health problems; 4. The patient or his/her family member (if the patient has communication problems) is willing to cooperate with the investigation and sign the informed consent.

Exclusion criteria

Exclusion criteria: Elderly patients receiving community day care services.
