Inclusion criteria

Inclusion criteria: (1) Aged >= 18 years; (2) The patients with ischemic stroke for the first time met the diagnostic criteria revised by the fourth national cerebrovascular disease conference, and were confirmed by head CT or MRI; (3) Primary school or above education level, basic reading and writing ability and wechat software use ability; (4) The expected length of stay was more than 7 days and adequate health education was provided; (5) Patients who are expected to use two or more of the antihypertensive, hypoglycemic, lipid-lowering and antiplatelet aggregation drugs at discharge; (6) Patients or their families signed informed consent.

Exclusion criteria

Exclusion criteria: (1) Patients receiving thrombolytic therapy or intravascular intervention; (2) Patients with unclear consciousness and primary mental illness; (3) Accompanied by other serious organ dysfunction or medical diseases.
