Inclusion criteria

Inclusion criteria: 1. Aged >=18 years old; 2. Sign the informed consent; 3. Acute ischemic stroke in the first episode; 4. Mild to moderate stroke and mRS<=2 when expected to be discharged from hospital.

Exclusion criteria

Exclusion criteria: 1. Patients who cannot communicate with each other normally; 2. Patients with other heart, brain, kidney and other serious complications; 3. Patients who cannot use smart phones.
