Inclusion criteria
Inclusion criteria: 1. Patients with essential hypertension; 2. Resident residents of the community; 3. Those who have signed up for the community health service center and voluntarily participate in the project can go to the designated medical institutions for hypertension diagnosis and treatment within one year after the implementation of the project, and then refer to the designated higher-level medical institutions if necessary; 4. Those who are not expected to take part in the project due to vacation, going abroad and other reasons within one year; 5. Subjects aged 40-80 years old; 6. Subjects with medication compliance score <= 7.
Exclusion criteria
Exclusion criteria: 1. Patients with hypertension emergencies; 2. Patients with acute myocardial infarction (< 3 months) and acute stroke (< 3 months); 3. Pregnant women and lactating women; 4. Patients who are not easy to cooperate with (intelligence, hearing and physical activities are obviously impaired); 5. The life expectancy of patients with severe diseases is less than one year; 6. Medical staff.
