Inclusion criteria

Inclusion criteria: Coexistence >= 2 chronic diseases lasting 6 months or longer; Stable condition, non-acute stage and hospitalization; 18 years old or above, who are willing to participate in this study and can cooperate, and sign informed consent.

Exclusion criteria

Exclusion criteria: With mental diseases such as unclear consciousness and no self-knowledge.
