Inclusion criteria
Inclusion criteria: (1) Aged >= 18 years; (2) Meet the diagnostic criteria for angina, myocardial infarction, heart failure, cerebral infarction, and cerebral hemorrhage; (3) The patient voluntarily signs a written informed consent form and can cooperate to complete the research work.
Exclusion criteria
Exclusion criteria: (1) Those who are participating in other chronic disease management projects; (2) Severe psychiatric diseases, or mental disorders caused by other major diseases; (3) Severe aphasia, ignorance, or apraxia; (4) According to the investigator's judgment, the patient cannot complete the study or cannot comply with the requirements of the study (due to management reasons or other special reasons).
