Experimental group:1. Construct continuity nursing unit. It is composed of department director, head nurse, clinical nutritionist, 2 hemodialysis doctors with bachelor degree or above and ward nursing

Inclusion criteria
Inclusion criteria: 1. Patients with a clear history of diabetes mellitus; 2. The patients whose clinical manifestations and auxiliary examination results were confirmed as end-stage diabetic nephropathy; 3. Patients with renal insufficiency caused by diabetes should be excluded from other causes; 4. Patients receiving maintenance hemodialysis treatment; 5. Patients aged over 65.
Exclusion criteria
Exclusion criteria: 1. Patients with other serious diseases at admission; 2. Patients with severe complications such as acute, chronic infection and malignant tumor; 3. Patients with mental illness, unclear thinking and dementia; 4. Patients who have participated in other similar studies; 5. Patients who refused and were unable to complete the study.
