intervention group:Implement an evidence-based practice plan for self-care of patients with COPD based on Proms and evidence

Control group:Routine self-care plan for COPD patients

Inclusion criteria

Inclusion criteria: 1. Diagnosis of patients with gold in 2019; 2. Patients with stable COPD can basically take care of themselves; 3. Patients who can accept telephone follow-up; 4. Patients who are equipped with smart phones and willing to use them; 5. Patients with basic understanding and reading ability, no mental disorders, and have signed informed consent.

Exclusion criteria

Exclusion criteria: 1. Patients who have undergone lung volume reduction, lung transplantation or pneumonectomy; 2. Patients with limb dysfunction; 3. Patients with cognitive impairment; 4. Patients living in other provinces who cannot be followed up.
