Inclusion criteria
Inclusion criteria: Inclusion criteria for elderly patients with type 2 diabetes: 1. Aged 60 to 80 years; 2. Meet the diagnostic criteria of diabetes recommended by WHO in 1999 and diagnosed as type 2 glycosuria for 1 year or more; 3. HbA1c test >= 7% in the last three months; 4. Patients have resided in Beijing for more than one year; 5. Patients are conscious and have complete cognitive and behavioral abilities ; 6. They are willing to nominate 2 members of the social network who are most closely related to each other, and the members live in Beijing; 7. Informed consent, patients and social network members are willing to participate in this study. The inclusion criteria of social network members were as follows: 1. Aged 18-80 years; 2. Good physical condition; 3. Education level above primary school; 4. Clear consciousness and complete cognitive and behavioral ability; 5. Informed consent.
Exclusion criteria
Exclusion criteria: Exclusion criteria for elderly patients with type 2 diabetes: 1. Complicated with other serious diseases, such as severe heart, liver, renal insufficiency, respiratory failure and malignant tumor; 2. Plan to stay in another place for more than 4 weeks in the next 3 months. Exclusion criteria for social network members: Plan to stay in other places for more than 4 weeks in the next 3 months.
