Inclusion criteria
Inclusion criteria: 1. Permanent population, 35 years old above; 2. Diagnosed with one or more of the following diseases: stroke, hypertension, atrial fibrillation and valvular disease, hyperlipidemia, diabetes mellitus and transient ischemic attack; 3. At least one of the patients and their family members has a smartphone and can independently complete the basic operation of smartphone APP; 4. The agreed to cooperate with the investigation and follow-up, and informed consent.
Exclusion criteria
Exclusion criteria: 1. The patients with serious or life-threatening diseases; 2. Short-term resident patients from other places; 3. Patients or high-risk groups who migrate to other cities during the project; 4. Patients or high-risk groups and their families do not have smartphones or can not independently complete the basic operation of smartphone APP; 5. Patients participate in similar intervention projects during the same period.
