65 Years to 110 Years
Inclusion criteria
Inclusion criteria: 1. Patients with age >= 65 years old; 2. Patients with stable cardiovascular and cerebrovascular diseases; 3. Patients with frail score >= 3; 4. Patients who understand the content of the scale, can communicate normally and cooperate to complete the assessment test; 5. Subjects who participated in this study voluntarily.
Exclusion criteria
Exclusion criteria: 1. Patients with end-stage disease and life expectancy <= 12 months; 2. Patients with severe cognitive impairment or complete disability and bedridden; 3. Patients with acute liver, kidney, tumor and hematopoietic diseases; 4. Patients who are participating in other studies similar to this study.
