Interventions

Intervention Group (ADHM cohort):Administrative-driven hierarchical management (ADHM) of atrial fibrillation with ACC-to-ABC pathway
Control Group (UC cohort):Usual care (UC): matched with the ADHM cohort with an 1:5 ratio. Only observation, no intervention.

Inclusion criteria

Inclusion criteria: 1. Atrial fibrillation participants (aged >=18 years) in all community medical and health centers in Changning District, Yangpu District, and Xuhui District of Shanghai; 2. Atrial fibrillation populations in other administrative districts of Shanghai with age, gender, and other factors 1:5 matching to the ADHM cohort (using the electronic information platform of the Shanghai Municipal Health Commission for matching).

Exclusion criteria

Exclusion criteria: 1. Non resident of Shanghai; 2. Resident without government-issued medical insurance; 3. Participants without information in the database until the research deadline; 4. Pregnant or have pregnant plan; 5. Life expectancy < 3 months; 6. Patients requiring immediate hospitalization or already receiving inpatient care at the time of enrollment; 7. Refused to participate.
