Inclusion criteria

Inclusion criteria: Acute or progressive dyspnea, which started within one week after the predisposing factors were identified. Chest imaging (X-ray plain film / chest CT) showed bilateral lung infiltration, which could not be fully explained by pleural effusion, lobar / whole lung atelectasis and nodules, or by heart failure or fluid overload. When hypoxemia showed peep >= 5cmh2o, PaO2 / FiO2 <= 300 mm. The patients were classified according to the degree of hypoxemia: Mild: 200 < PaO2 / FiO2 <= 300 mmHg; Moderate: 100 < PaO2 / FiO2 <= 200 mmHg; Severe: PaO2 / FiO2 <= 100 mmHg.

Exclusion criteria

Exclusion criteria: 1. Pregnancy or active lactation; 2. brain death; 3. terminal-stage cancer or other disease; 4. a decision to do-not-resuscitate; 5. treatment with corticosteroids or immunosuppressive drugs; 6. enrollment in another experimental treatment protocol; 7. severe chronic obstructive pulmonary disease; 8. or congestive heart failure.
