Inclusion criteria

Inclusion criteria: (1) Men and women aged >= 60 years; (2) Type 2 diabetes (3) Raised systolic blood pressure:systolic blood pressure >= 140 mmHg on at least 1 anti-hypertensive medication; (4) Increased risk of cardiovascular disease: with history of cardiovascular disease, and/or subclinical cardiovascular disease, and/or eGFR 30-59 ml/min/1.73 m2; (5) Sign informed consent.

Exclusion criteria

Exclusion criteria: (1) Known secondary cause of hypertension; (2) One minute standing systolic BP 2.0 mg/dl; (9) Previous diagnosis of polycystic kidney disease or glomerulonephritis; (10) Any factors judged by the clinic team to be likely to limit adherence to interventions; (11) Currently participating in another intervention study; (12) Currently living with another participant; (13) Pregnancy, currently trying to become pregnant, or of child-bearing potential and not using birth control.
