Inclusion criteria

Inclusion criteria: 1. APACHE II score >=20; 2. Terminal cancer with multi-organ metastasis; 3. Severe intra-cerebral hemorrhage; 4. End-stage COPD and respiratory failure; 5. Advanced malignancy and shock.

Exclusion criteria

Exclusion criteria: 1. Patients whose families with language barrier; 2. Patients estimated to be discharged within 3 days (insufficient time for FCSDM intervention); 3. Patients whose families reject to sign the research consent.
