Inclusion criteria

Inclusion criteria: Inclusion criteria of patients: 1. Admitted to a stroke due to stroke and caused ADL dependence, those with normal ADL 2 weeks before admission; 2. Aged >= 60 years; 3. Live with family members; 4. Informed consent to the investigation. The inclusion criteria of caregivers: 1. Family members or relatives of patients who provide unpaid care services; 2. The patient's primary family caregiver, with the longest caring time as the standard; 3. Aged >= 18 years; 4. Able to communicate effectively with researchers, and informed consent.

Exclusion criteria

Exclusion criteria: Exclusion criteria for patients: 1. Living alone or not living with family members; 2. Mental retardation; 3. Suffer from severe cognitive impairment; 4. The mentally ill. Exclusion criteria for caregivers: 1. Those who have an employment relationship with the patient or the patient's family members; 2. Do not live with the patient after discharge.
