Inclusion criteria

Inclusion criteria: (1) Aged >=18 years; (2) Discharge time >= 3 months; (3) Good listening, speaking, reading and writing skills; (4) No cognitive impairment or mental illness; (5) Patients or their family members can use smart phones; (6) Volunteer to participate in this study.
