Inclusion criteria

Inclusion criteria: 1. Age: 60-85 years old; 2. According to the diagnosis standard of China's hypertension prevention and control guidelines in 2018, patients who have been diagnosed with hypertension are not limited to the types and quantities of combined diseases, such as diabetes, hyperlipidemia, stroke, coronary heart disease and atrial fibrillation; 3. Patients who use more than 5 drugs in combination; 4. Patients sign informed consent.

Exclusion criteria

Exclusion criteria: 1. The patient and his / her caregiver are unable to maintain stable contact with the telephone and remote management platform; 2. Patients who are difficult to communicate; 3. Patients living in other places for a long time.
