25 Years to 75 Years

Inclusion criteria

Inclusion criteria: Suffering from at least one of the following diseases: 1. Patients with at least one of the following diseases: (1) Coronary heart disease (positive coronary angiography, CTA diagnosis or exercise load test); (2) Hypertension; (3) Stable heart failure (NYHA I-III and stable cardiac function grading in recent 2 weeks); (4) Hyperlipidemia (including hypertriglyceridemia and hypercholesterolemia); (5) Prediabetes (impaired fasting glucose, impaired glucose tolerance) or diabetes; 2. Patients aged 25-75 years (including the threshold), regardless of gender; 3. The education level shall be primary school culture or above, and the patients who can understand the questionnaires and play the self-made elastic band cardiotonic exercise CD.

Exclusion criteria

Exclusion criteria: 1. Patients with severe mental and psychological diseases; 2. Patients with other serious diseases (severe pneumonia, cerebral hemorrhage or subarachnoid hemorrhage, severe liver function and renal function damage, etc.); 3. Patients with implantable devices (pacemaker or ICD, CRT); 4. Patients with decompensated or acute heart failure; 5. Patients with serious arrhythmia (ventricular tachycardia, ventricular flutter and fibrillation, supraventricular tachycardia, rapid atrial fibrillation, severe bradycardia - less than 50 times / min in quiet state, and atrioventricular block of degree II or above); 6. Patients with severe diseases (such as lumbar spinal stenosis and severe degenerative knee arthritis) with limited exercise test and training; 7. Patients with diseases that lead to a life expectancy of less than 1 year (such as malignant tumors).
